The first step of Kinsale’s appointment process is completing our Producer Questionnaire. You can complete the questionnaire online or download the Producer Questionnaire to fill out.
Once you complete the form and attach the necessary documents, you will be able to provide an e-signature, click “Continue,” and the form will be sent to us automatically.
Be sure to have your entity/individual producer license(s) for all states, copy of latest financial statement, Fidelity Declarations plan, and E&O Declarations page. available to either upload online or email as an attachment.
